Certificate Samples

Each certificate is in 2 parts; a cover which indicates the name and site of the certificate holder and the schedule which includes the serial numbers, the medicinal product authorised and the treatment or diagnostic test for which this can be used. In the case of research the title of the research project is also included in the schedule.
